Vaccinations

childhood vaccinations

All childhood vaccinations are available at the practice.  All the doctors encourage parents to have their children vaccinated in line with the accepted (NH&MRC) guidelines.  Please let the office staff know when making your appointment if it is for vaccination.

adult vaccinations

Influenza vaccines are available each year.  We recommend that everyone over the age of 65 and those with chronic illnesses, such as asthma, diabetes, and heart disease should have a vaccination.

If you wish to have a vaccination you should discuss it with your doctor.

travel vaccinations

For overseas travel you should generally present at least 6 weeks prior to departure for optimal cover.

Appointments must be made with the doctor to discuss individual needs.  Occasionally vaccines are not immediately available and a short delay may ensue.

Chronic Disease management & ENHANCED PRIMARY CARE

Prospect Medical Centre has committed to undertaking Chronic Disease Management and Enhanced Primary Care programs to achieve the best possible outcomes for our patients who are living with chronic or ongoing illnesses.  Our doctors and nurses work with eligible patients to develop ongoing health management plans.

GP Management Plan

A GP Management Plan involves you, your doctor and a practice nurse who, with your consent and assistance, will form a written plan of management outlining your needs and that will best suit your needs.  Together with you doctor you establish goals and work towards them to improve your overall health and wellbeing.

Health Assessments

Health Assessments are also available for our patients who are aged 75 years and over.  Health Assessments enable the practice to gain a more detailed overview of health needs and services for this age group.
